Job Summary:
To provide continuous medical supervision in consultation with the patient, family and other members of the Home Care team then develops and prescribes home care plan of treatment.
Principal Duties:
1. Managing of medical problems.
2. Ensuring initial eligibility of the patient to go home by Performing patient's medical assessment and based on the inclusion/exclusion criteria before requesting Home Care Team evaluation.
3. Identifying of Home care needs of the patient.
4. Establishing or approving a plan of treatment with identification of both short-and-long term goals.
5. Evaluating of new, acute, or emergent medical problems based on information supplied by other team members.
6. Communicating with the patient and other team members and with physician consultants.
7. Following up all aspect of care with multidisciplinary team members on regular basis to ensure continues flow.
8. Ordering all patients medical equipment recommended by multidisciplinary team.
9. Referring the patient to any other specialty before and after patient discharge home as required.
10. Admitting patient to hospital as needed.
11. Performing patients home visits as appropriate.
12. Making house calls as appropriate.
13. On-call coverage by a physician.
Educational Requirements:
1. Graduate of MBBS from a medical school recognized by the Saudi Commision for Health Specialties and acceptable to KFMC.
2. Successful completion of three (3) or more years in a sub-specialty residency training program recognized by SCHS and acceptable to KFMC
3. Certified by the appropriate board recognized by SCHS and acceptable to KFMC (e.g. Saudi Board, Arab Board, American Board, Canadian Board).
4. One or more years of sub-specialty training in a recognized program outside Saudi Arabia
5. Training and post certificate experience equal to or greater than six (6) years in a health care facility recognized by KFMC
6. Independent practice in the sub-specialty
7. Should meet the SCHS criteria within one year of application
Training:
1. One year of internship in a recognized training program.
2. Subspecialty fellowship training in the related subspecialty in a
recognized training program.
Other Requirements:
1. Knowledge in Home Health Care
2. License or accreditations for medical staff
3. Saudi Commission for Health Specialties registered
